Paperwork
For your first visit we will require some information about you and your health. You may download and print copies of the registration and medical history forms and complete them. You can bring the completed forms with you on your visit or e-mail them to us ahead of time. You may also complete the forms in our office on the day of your visit. Please plan to arrive 15 minutes early to allow sufficient time for paperwork.
Parking
We share underground parking with Swedish Medical Center Parking Garage at the main entrance of the hospital at Broadway and Columbia. In the garage, follow the signs to Heath Building. Parking is free for 0-30 minutes and then $4.00 for 30-60 minutes.
At the initial visit, Dr. de la Torre will typically spend 30-45 minutes to discuss your goals and treatment. Subsequent visits should only take 15-20 minutes. Payment for Botox injections are charged at time of service based on number of units injected. Cash, check and credit cards are accepted.
Cancellation policy
We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 48 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, we reserve the right to ask for base pre-payment by credit card prior to rescheduling another appointment for you.
Privacy
The privacy of your personal and medical information will be fully maintained by our office.
